I'm using a FTU Kit by KittzKreationz 

It's called Toxic City Toxic City

Open a 600x600 image, 

I'm using two of his tubes, you can use any that will match the kit...

Open T-42 copy and paste as a new layer, move it to the upper left hand side of the tag

Open T-P4 using your selection tool, put the T-P4 as the background layer

make a square behind the caution element

Add noise of 26 on the Add Noise function

Open T-9 Skull, and position it  over the T-42, add a drop shadow (mine is 5/5)

Open your tube, and copy paste it in a new layer pull it slightly over T-42

Open T-5 copy, re-size by 45% pull it over your tube, ( over her left ankle)

Open T-33 copy, paste, re-size by 40% mirror the element, pull it to the lower right hand side of the tag, over her right foot..

Open T-10 copy, paste, over T-42 and it will be your back drop for the tube, add a gradient glow of white to make the city stand out better.. 

You can add other flares to the tag, to make it your own. This is just a guide line for you..

I'm using the font AsukaCAPS 

i'm using a size of 36 with a white foreground, and layer 4 the T-P4, you will 

have to copy this layer and paste as a new image...

Add a gradient glow of red, then default drop shadow

Add your copyright's make sure you credit who ever the tube belongs too.. 

Please do not rip my tag's or claim them as your own.
